§ 18.1305. Application.

  • The application for assistance for payment of County connections costs and eligible costs for eligible work shall be signed by each person with an ownership interest in the property and shall be submitted to the Director of Public Works on a form authorized by the Director of Public Works. The application shall contain the following information and acknowledgments:

    (a)

    The property owner's name and mailing address;

    (b)

    The address and tax account number of the property which is to be connected;

    (c)

    A petition to incorporate into the metropolitan district the property to be connected to the public water system unless the property is in the metropolitan district;

    (d)

    A list of the costs for which the property owner is requesting assistance;

    (e)

    A statement acknowledging that the property owner is responsible for the selection of the contractor and for the performance of all work needed to connect to public water and that the County does not warrant the work performed by the contractor;

    (f)

    A statement that the property owner agrees not to connect any on-site well to the public water system and will comply with all applicable laws and regulations;

    A statement that assistance provided pursuant to this subtitle is subject to the availability of funds.
